YPL PARTICIPATION WAIVER, RELEASE OF LIABILITY, MEDICAL CONSENT, MEDIA RELEASE & CODE OF CONDUCT

This agreement must be read and signed by a parent or legal guardian ("Parent") on behalf of each minor participant ("Player") before the Player may participate in any YPL league, practice, placement session, championship, event or related activity ("Activities").

1. ACKNOWLEDGMENT OF RISK

I understand that pickleball is a physical sport and that participation in the Activities involves inherent risks, including but not limited to: falls, collisions with other players, paddles, balls, nets or court surfaces; sprains, strains, fractures, cuts, bruises, concussions and other injuries; heat-related illness; and, in rare cases, serious injury, permanent disability or death. I understand these risks exist on and off the court, during play, warm-up, travel between courts and at facility premises. I knowingly and voluntarily accept these risks on behalf of the Player.

2. RELEASE OF LIABILITY AND INDEMNITY

In consideration of the Player being permitted to participate, I, on behalf of myself, the Player, and our heirs, assigns and personal representatives, release, waive and discharge YPL, its parent and affiliated entities, owners, officers, directors, employees, contractors, coaches, officials, volunteers, sponsors, and the owners and operators of host facilities (including Pickle & Pint, Dill Dinkers, Tampa Pickleball Crew and PB Kingdom) (collectively, the "Released Parties") from any and all claims, demands, losses or liability for injury, illness, death or property damage arising out of or related to the Player's participation in the Activities, including claims arising from the ordinary negligence of the Released Parties, to the fullest extent permitted by Florida law. I agree to indemnify and hold harmless the Released Parties from any claim brought by or on behalf of the Player or by a third party arising from the Player's conduct.

Nothing in this release waives claims for gross negligence, willful misconduct or any liability that cannot be waived under applicable law.

3. MEDICAL CONSENT AND AUTHORIZATION

I represent that the Player is physically fit to participate and has no condition that would make participation unsafe, except as disclosed in writing at registration. In the event of injury or illness, I authorize YPL staff, coaches and officials to obtain emergency medical treatment for the Player, including first aid, transport by ambulance and treatment by licensed medical providers, if I cannot be reached. I accept financial responsibility for any medical treatment provided. I will keep my emergency contact information current with YPL.

4. CONCUSSION AND HEAT AWARENESS

I acknowledge receipt of YPL's concussion and heat-illness guidance. Any Player showing signs of a concussion will be removed from play and will not return until cleared in writing by a licensed healthcare provider. I will not send the Player to participate while ill or injured.

5. PHOTO, VIDEO AND MEDIA RELEASE

I grant YPL and its designees the irrevocable right to photograph, film and record the Player during the Activities and to use the Player's name, likeness, image, voice, team, division, rating, ranking and results in any media now known or later developed, including websites, social media, rankings boards, broadcasts and promotional materials, without compensation. I may opt out of promotional use by unchecking the media release box at registration or by written notice to info@youthpickleballleague.com. Opting out does not remove the Player from rankings, standings or results displays, which are part of league operations.

6. CODE OF CONDUCT

Players agree to: compete with effort and sportsmanship; respect teammates, opponents, coaches, officials and facility staff; accept officials' calls without argument; use no profanity, taunting, bullying or physical aggression; follow facility rules; and arrive on time in YPL uniform.

Parents and spectators agree to: support all players positively; never coach from the sideline, confront officials, coaches or other families, or enter the court during play; model respect; and follow facility rules, including alcohol policies at host facilities.

Coaches and staff agree to: pass a background check; complete YPL training; never be alone with a Player out of view of others; communicate with Players only through Parents or approved league channels; and report any safety concern immediately.

Violations may result in warnings, ejection from a session, suspension or removal from the league without refund, at YPL's discretion.

7. RULES, ELIGIBILITY AND DATA

I agree that the Player will abide by YPL rules, formats and division placements. I confirm the Player's date of birth is accurate; misrepresentation of age results in removal. I understand that match results, YPR rating and rankings are league records maintained by YPL and may be displayed publicly with the Player's name, team and division.

8. GENERAL

This agreement is governed by the laws of the State of Florida. Venue for any dispute lies in Pinellas County, Florida. If any provision is found unenforceable, the remainder stays in effect. This agreement remains in effect for all YPL Activities in which the Player participates unless revoked in writing, and applies to each Player listed at registration.

SIGNATURE

I have read this entire agreement. I understand I am giving up substantial legal rights, including the right to sue. I sign it voluntarily on behalf of myself and the Player.
